INTENSIVE CARE Pre-procedure ultrasound increases the success and safety of central venous catheterization

چکیده
Editor’s key points † It is recommended that ultrasound (US) is used to facilitate cannulation of the internal jugular vein (IJV). † This observational cohort study compared pre-procedural US with landmark techniques for IJV cannulation under general anaesthesia. † Pre-procedural US was associated with higher success rates, shorter cannulation times, and fewer complications. † These differences occurred with both experienced and inexperienced operators. Background. Real-time ultrasound (US) in central venous catheterization is superior to preprocedure US. However, moving real-time US into routine practice is impeded by its perceived expense and difficulty. Currently, pre-procedure US and landmark (LM) methods are most widely used. We investigated these techniques in internal jugular vein (IJV) catheterization in respect of operator experience, complications, and risk factors.

Ultrasound-guided central venous access
Central venous catheterization is a critical component of management for the critically ill patient in the operating room and intensive care unit. When using ultrasound techniques for central venous access, access is achieved with fewer attempts, a reduced incidence of carotid artery punctures or 'hits', an increased success rate, and a decreased duration of procedure compared to the traditiona...

متن کاملReply from the authors Pre-procedure ultrasound increases the success and safety of central venous catheterization
We really appreciate the interest of Smith and McCarthy in our study. They raised two issues which we would like to comment on. First, the authors’ point is well taken and they are absolutely right with their critique that we did not list normal coagulation and platelet count/function as specific inclusion criteria. Although not listed, we can guarantee that the following criteria were fulfille...
متن کاملA Missing Guide Wire After Placement of Peripherally Inserted Central Venous Catheter
BACKGROUND Central venous catheterization is a common tool used in critically ill patients to monitor central venous pressure and administer fluids and medications such as vasopressors. Here we present a case of a missing guide wire after placement of peripherally inserted central catheter (PICC), which was incidentally picked up by bedside ultrasound in the intensive care unit. CASE REPORT A ...
متن کاملCorrelation of Ultrasonographic Measurement of Inferior Vena Cava Collapsibility Index with Central Venous Pressure in Diagnosis and Management of Neonatal Shock
Background: Neonatal intensive care unit (NICU)Conventionally, Central Venous Pressure (CVP) monitoring has beenused by intensivists to measure intravascular volume. However, it is an invasive procedure resulting in many complications. Non-invasive ultrasonographic measurement of inferior vena cava collapsibility index (IVC-CI) is a promising alternative. Therefore, this study wasconducte...
